An illustration of the method used in the study. To describe, understand and answer the research question, three different guiding questions were used in the content analysis of the incident investigations. In the content analysis, data from the internal and external incident investigations were linked together respectively. All targets for intervention in all investigations were identified, examined and coded together with plans for follow-up or other intentions. The method used gave both quantitative and qualitative results for further interpretation
